“In the beginning, it seemed like a monumental task. I thought, ‘How can just one average guy spread the word about living healthy with diabetes and why would anyone be inspired by me?’ I quickly learned that people were interested in my story because I had walked in their shoes and have the same health concerns that they have.” —Mark House, New Jersey

Follow our Health Monitor CHA Facebook Page to share your own experience and help others. We are passionate individuals who either have medical backgrounds, or have had a positive experience and want to share our stories. Please respect the privacy of those who are participating. This is not a forum to seek or give medical advice. It is merely a tool to help guide conversations/share testimonials. |

![]() HMN is developing a series of HIPPA compliant Apps offered by doctors to support patients after they leave the practice. App users will receive daily reminders to help track symptoms, receive education, e-meet others with similar condition and more! Doctors can view information on a dashboard, but most of the communication happens by a patient bringing in the report themselves either printed out or on your mobile device. The aim of our Apps is to improve patient and provider engagement and adherence and allowing users to clearly log in and communicate symptoms. This makes follow up appointments much more productive.
