What’s a Community Health Ambassador (CHA)?

Health Monitor’s CHAs are passionate, dedicated volunteers who spread messages of hope, healing and education to people living with cancer, diabetes, rheumatoid arthritis and other health conditions.

What will you do?

Participate in community events like health fairs, hospital open houses, and health events at houses of worship, senior centers and more. We won’t send you out empty-handed! Along with copies of our trusted patient-education publications, we’ll provide you with an official Community Health Ambassador badge, personalized fliers and other tools.

HMN is developing a series of HIPPA compliant Apps offered by doctors to support patients after they leave the practice. App users will receive daily reminders to help track symptoms, receive education, e-meet others with similar condition and more!

Doctors can view information on a dashboard, but most of the communication happens by a patient bringing in the report themselves either printed out or on your mobile device.

The aim of our Apps is to improve patient and provider engagement and adherence and allowing users to clearly log in and communicate symptoms. This makes follow up appointments much more productive.
